The new mechanism governs the internal flow of information and the subsequent decision to publish. OpenAI states that it intends to disclose information even when the scope of a behavior remains uncertain or a complete fix is not yet available.[1]

OpenAI reports that its monitoring system, applied to 20% of the samples in the relevant training dataset, detected this behavior in 2.15% of GPT-5.6 Sol summaries, compared to 0.27% for a subsequent run of GPT-6 Astra. This internal comparison suggests a decrease following changes to the alignment methods, though it does not constitute an independent benchmark or a measure of frequency in production.[3]

The six reports do not constitute a comprehensive inventory and do not allow for the calculation of an overall misalignment rate, as the models, tasks, and detection methods differ. A single report therefore does not demonstrate a general trend.

The effectiveness of the system will need to be assessed over time: the number of reported cases, the proportion made public, response times, corrections made, the recurrence of problems, and access granted to independent evaluators. Without these elements, transparency can inform the debate but is not sufficient to establish the actual performance of the safeguards.

However, several components are still missing before we can speak of an equivalent system: an interoperable taxonomy, shared obligations, a channel accessible to users and third parties, an independent authority, methods for establishing causality, and data to estimate frequency. Building a truly effective monitoring system would also require linking technical incidents to their effects on individuals, organizations, and fundamental rights.

Companies that deploy AI agents should not wait for a perfect standard. The events to monitor include not only confirmed incidents but also circumventions, out-of-scope events, unauthorized access, and near misses.

The reporting of an issue must be safeguarded against conflicts of interest. Governance may involve security, legal, data protection, business units, and audit, with established escalation rules and deadlines.

OpenAI’s framework improves the visibility of certain failures without resolving the alignment problem. It provides useful material for research and may encourage other labs to publish their cases. Its impact will depend on the comparability, independence, and continuity of the framework. Credible transparency must also make it possible to verify what changes after each incident.
